What to Expect (Job Responsibilities):
Assist members in connecting to community and care resources in a cost-effective manner
Support coordination of community outreach resources and promote awareness of services
Document and maintain community resources to align with health plan standards
Conduct non-clinical assessments to refer members to appropriate care/services
Perform telephonic and/or in-person outreach to connect with hard-to-reach individuals and families
What is Required (Qualifications):
High School diploma or GED
1 – 2 years of related experience
Working knowledge of Social Determinants of Health (SDOH) barriers
Strong communication and interpersonal skills
Ability to comply with all policies and standards
How to Stand Out (Preferred Qualifications):
Experience in community outreach or health education
Familiarity with care management processes
Bilingual skills to assist diverse populations
