Our team lifts each other and those we serve with consistency, authenticity, adaptability, and a resolute commitment to equity in health care./ppPrimarily responsible for providing HIV/STI/Hepatitis testing, community outreach, individual intervention, and group facilitation to target populations in the community and agency./ppstrongEssential Duties and Responsibilities:/strong/pulliConduct HIV/STD and Hepatitis C screenings which include collection of blood samples and urine specimens adhering to universal precautions./liliEngages, counsels, manages, motivates and effectively works with high-risk, vulnerable patient population, including chemically dependent persons./liliAssesses clients educational and risk-reduction needs and assists clients in developing risk reduction strategies for reducing high-risk behavior./liliDevelops and delivers disease prevention, intervention and health promotion presentations to various audiences in the community./liliEngages in street outreach in public places in targeted population areas which consists of communicating information about the availability of HIV services./liliProvides HIV education, including basic information regarding prevention of transmission./liliDocuments all activities and services provided and submit required documentation in a timely manner./liliAccurately enters all data as required into various databases./liliProvide linkage and navigation for health care, support services and risk reduction activities./liliFacilitate groups and individual interventions including screening, testing, life skills, social rehabilitation and risk reduction./liliAttend and participate in staff meetings, program meetings, and staff training as determined by supervisor./liliFollow through on verbal and written communication from the Program Manager and follow policies and procedures of the program./liliPerforms other duties as assigned./li/ulpstrongKnowledge, Skills, and Abilities:/strong/pulliHours vary and you must be able to work flexible hours including evenings and weekends/liliMust have a strong comfort level working with the target population as well has working some late nights at locations with high traffic of target population./liliKnowledge of community / County resources/liliExcellent verbal and written communication skills with the ability to communicate with diverse populations in public settings/liliThe ability to effectively engage and work in a stressful environment with high-risk, vulnerable client population and people in crisis./liliAbility to effectively communicate with people at all levels and from various backgrounds./liliAn ability to maintain a strong sense of respect for confidentiality involving both clients and fellow employees./liliAbility to establish and maintain effective working relationships with employees, other agencies and the public/liliAbility to follow written and verbal instructions and meet established deadlines/liliBasic computer skills required: Microsoft outlook email, office/liliBasic math skills required: addition, subtraction, multiplication, and division/liliAbility to work independently with minimal direction/liliKnowledge of basic risk reduction methods/liliBasic Assessment Skills/liliAbility to formulate interventions/recommendations based on assessment/liliBasic individual intervention skills/liliBasic Group facilitation skills./liliAbility to legally operate a motor vehicle and provide own transportation./liliSpanish speaker preferred/li/ulh2Education and Work Experience:/h2ulliHigh-School Diploma or GED equivalent required/liliTwo years of Substance Abuse Training and/or one year of job-related experience preferred./liliKnowledge of HIV/AIDS and conducting Intervention/Prevention services with minority and at-risk preferred./li/ulpstrongCertificates and Licenses:/strong/pulliHIV 501 State of Florida Certification/liliCertification in Phlebotomy preferred./liliValid Florida Drivers License and proof of insurance required.
