Job Description Under the supervision of the Transition of Care Nurse Manager, the PACE RCFE Supervisor, LVN will be a working supervisor role, ensuring clinical case management and guidance and instruction to the RCFE Placement Navigation team, ensuring appropriate permanent Residential Care Facility for the Elderly Placement for PACE participants as approved by the Interdisciplinary Care Team. In addition, the PACE RCFE Supervisor, LVN will provide member advocacy, ensure benefit establishment, linkage to stable housing, and all other supportive services as needed. The PACE RCFE Supervisor, LVN will ensure individualized participant support, ensuring the RCFE Placement Navigator members develop a plan to address their barriers, access benefits, and maintain and sustain permanent housing. The PACE RCFE Supervisor, LVN will work with different PACE disciplines and various leaders to ensure all clinical consideration is included in the provision of the highest level of care to PACE participants.

Essential Functions of the Job:

Responsible for the overall case load assignments and completion of the RCFE Placement teams daily tasks.

Create and conduct trainings for RCFE team staff.

Provide overall supervision for employees within this team, including hiring and performance management.

Ensure staff have the knowledge and tools to work with participants as identified by the PACE Interdisciplinary Care Team who require Residential Care Facility for the Elderly Placement, assist them with RCFE referrals, resources, advocacy, and case management services as needed.

Provide support and guidance for RCFE Placement Navigators, ensuring they are assisting participants in navigating services and to obtain stability.

The RCFE Placement Navigator Supervisor will be responsible for coordination of services, information, education, and referrals to Residential Care Facilities for the Elderly.

Assist staff members in their search for Residential Care Facility for the Elderly placement and ensure completion of appropriate documentation.

Conduct routine audits of work and ensure closing the loop.

Establishes updates, maintains, and communicates a list of available Residential Care Facility for the Elderly opportunities at least once a month.

Ensures effective service delivery by keeping up with any referrals, follow up, etc. of all Residential Care Facility for the Elderly opportunities.

Respond to referrals and member’s request for case management assessment and intervention within required response time.

Maintain member related data tracking systems and document all efforts in EPIC.

Assists participants in completing any required Residential Care Facility for the Elderly applications and accompanying them to appointments to view Residential Care Facility for the Elderly options, etc., as needed.

Prepare case-related reports including outcomes, successes, and challenges.

Generate member data for monthly reporting.

Complete follow-up and retention services and provide back-up documentation in client file.

Outreach to community and Residential Care Facility for the Elderly administrators to identify new and existing opportunities and build strong relationships to better assist members in accessing Residential Care Facility for the Elderly placement.

If required to support Social Services Department to carry out direct care coordination duties with patients (e.g. educating patients on self-management skills, coordination/oversight of specialty care referrals, and facilitating linkages to social supports). Provides timely updates on cases to Transition of Care Manager and specific participants’ MSW and HHC

Travel as required to all different PACE sites (PACE Administration office, San Ysidro, Chula Vista, El Cajon, and Vista)

Demonstrate ability to inter-relate with physicians, nurses, patients, internal departments, outside agencies, and the public.

Demonstrate customer-focused service skills, and ability to hold employees accountable to high standards.

Knowledge of individual Residential Care Facilities for the Elderly and the levels of care that are provided within each facility.

Basic physical, psychosocial, and functional assessment skills.

Able to collaborate between organizational and community resources.

Thorough knowledge of appropriate long-term care and home care resources.

Able to document concise yet thorough documentation of participant’s housing assessment and care needs.

Demonstrated strong communication and customer service skills, problem solving, critical thinking, time management, organizational skills, and clinical judgment abilities.

Familiarity and ability to use computers as well as EHR’s.

Complies with all department, organization, and government policies & procedures.

Creates trainings as needed.

Attends meetings and trainings as required.

Job Requirements: Experience Preferred:

One (1) year experience as a supervisor, LVN within the past two (2) years.

One (1) year experience providing resources to clients in a medical setting.

One (1) year previous experience with social services and Residential Care Facility for the Elderly placements.

Education Required:

Completion of an accredited LVN program.

Certifications Required:

Current and active License as a Vocational Nurse within the state of California

BLS certification (American Heart Association)

Verbal and Written Skills Required to perform the Job:

Ability to read, write and comprehend medical information and terminology.

Ability to effectively present information, both verbal and written.

Demonstrated knowledge of the RN scope of practice.

Good interpersonal skills.

Superb organizational ability and exceptionally analytical.

Technical Knowledge and Skills Required to Perform the Job:

Experience and knowledge with Microsoft Office software.

Epic and NextGen experience a plus

Equipment Used: Working Conditions and Physical Requirements:

Use of own vehicle to travel, hybrid office/work from home.

Employee will have frequent prolonged sitting and computer use.

Occasionally lifts supplies and equipment.

Handle emergency or crisis calls. Occasionally one is subjected to multiple calls, and inquiries. The employee is exposed to unpleasant elements (Accidents, injuries and illness).
