Healthcare Assistants (Inpatient Bicton, Apley and Community Based) As we prepare to reopen ourShrewsbury ward following a £3 million makeover there is a range of inpatientand community roles available.

We are keen to attract both newlyqualified individuals at the beginning of their careersand experiencedclinicians.

Working at Severn Hospice you aregiven the time to care and were looking for compassionate and committedHealthcare Assistants to join our Inpatient or Community Nursing Team.

With your friendly, empatheticand professional approach, you will work in a highly rewarding area of care,supporting patients under the direction of a Registered Nurse or NursePractitioner maintaining a consistently high standard of person-centred palliativecare.

As a valued member of our skilledand caring clinical team, you will play a vital role in meeting patientsholistic needs, ensuring comfort, safety and dignity, while also offeringongoing support to families and carers.

If the community team is moreyour area, as a valued member of our skilled and caring team, you willcontribute to a range of services including home-based care, coffee morningsand supporting outpatient clinics.

Main duties of the job To assist in the care of patients with a life limitingillness under the direction of a Registered Nurse and to maintain a highstandard of care. To support the patients and relatives through ongoingcommunication

Togenerate and maintain a friendly team spirit with all the staff and volunteersof the Hospice.

Toprovide a safe environment for patients, visitors, staff and volunteers.

Tosupport new HCAs into their duties

Ifrequested, to provide assistance on Hospice transport, ensuring safe transferof patients to and from the Day Services.

Tohelp embed the practices and culture of rehabilitative palliative care toeffectively support people with long term life-limiting conditions to livewell.

Community Based:

Our primary goal is to establish a tiered hospice communitycare model that can respond promptly to support our patients with theirevolving, complex needs, enabling them to remain safely at home if that is theirchoice.

HealthcareAssistants will support the Service through H@H day care, coffee mornings, andsupporting outpatient clinics, to ensure that high standards of care aredelivered and effective liaison and communication with key professionalsinvolved in the patients care are maintained.

Individuals will need todemonstrate the ability to work both independently and as part of amultidisciplinary team. They will needto be flexible in their approach to the variety of tasks they will undertake.

Job responsibilities PRINCIPAL RESPONSIBILITIES To support the trained members of the nursingteam in the implementation of individualised care programmes for patientscomfort, safety and dignity at all times (e.g. checking drugs with trainedmembers of staff/working within current risk assessments for individualpatients)

To perform designated nursing tasks when deemedcompetent theoretically and practically by a qualified nurse with theMentor qualification, as part of a planned programme of care (e.g.changing dressings, catheters, observing and documenting, testing urine,care of people using prescribed oxygen).

To report to the Ward Sister/ Ward Manager asappropriate for any changes in patients condition or relevant informationpertinent to patient care (e.g. dietary requirements).

To monitor and record observations (e.g.Temperature, Pulse, Respiration, Blood Sugar and Blood Pressure) oncompletion of relevant competency training.

To second check CD administration if training andcompetency achieved (within inpatient environment).

To record information on care given in patientheld records and other documentation to comply with legal and professionalrequirements.

To maintain confidentiality at all times.

To comply with all appropriate policies andclinical practices and protocols.

To attend such courses/lectures/in-servicetraining as indicated by needs of the service and own personaldevelopments.

To give verbal handovers to other members of theteam, including volunteers.

To escort patients to hospital appointments ortransfer to another place or home.

Assisting to carry outpersonal care after death incorporating family members if that is theirwish.

GENERAL To have anunderstanding of the role of other professionals involved in the patients careand provide support to them.

To work collaboratively alongside volunteers on a day-to-day basis. Togive support, guidance and instruction to the volunteer team members as andwhen required, incorporating handover at commencement of session

To give practicalsupport to relatives/carers/visitors.

At all times toact as ambassador for Severn Hospice to patients, relatives, to colleagues andmembers of the public.

To support theprocess of audit in an active way.Attending meetings and making a contribution.

To comply withHealth and Safety policy and fire regulations and to reportaccidents/incidents/potential risks immediately to a Senior Nurse.

To abide bysecurity procedures relating to staff, patients, visitors and their property.

Promote and incorporate equality and diversity in all aspects of theteams operations and in the public domain

To work flexiblyacross the organisation including rotation to all shifts (as appropriate) andthrough all departments as required to support the delivery of nursing carewithin the Hospice.

Supplementary Informationand Requirements Allnew Hospice employees are on a 6 month probationary period (excluding bank)

ADBS Disclosure will be requested in the event of an individual being offeredthe post.

Health and Safety In accordance with theHospices Health & Safety Policy all employed persons while at work arerequired by Section 7 of the Health & Safety at Work Act to take reasonablecare of their own health and safety and that of others who may be affected bytheir acts or omissions.

Employees are required toparticipate in the Hospices appraisal process as required.

Community Services: PRINCIPALRESPONSIBILITIES Make a positive contribution to excellent communications acrossthe whole organisation, ensuring effective communication both internally andexternally

Ensure that corporate values of the Hospice are actively promotedand followed at all times

Troubleshoot issues or problems with team members, referringconcerns to appropriate level when necessary

To provide basic nursing care and, in addition, interventionsdeemed appropriate and supported by training.

To be flexible and understanding to the needs of the patient andtheir family/carers.

To liaise with senior staff after each encounter to update themabout the patients condition.

To have an understanding of and be able to work within thephilosophy of palliative and terminal care.

To work in close liaison with all professionals, including thecommunity and Primary Health Care Team.

At all times maintain confidentiality concerning patients,relatives/carers, and other staff.

Ensuring the holistic physical, psychological, emotional,spiritual, and social needs of patients and their families, carers are met.

Ensuring that clear, precise documentationis carried out at all times about patient care.

Be trained in verification of death

PERSONAL To attend all mandatory training sessions as required.

To act at all times following all relevant policies and proceduresof Severn Hospice.

To report accidents/incidents/potential risks to appropriatesenior staff.

At act as an ambassador for the Severn Hospice to patients,relatives, carers, colleagues, and members of the public.

To attend such courses/lectures/in-servicetraining as indicated by needs of the service and own personal developments asidentified

Responsibilities and duties mayinclude other tasks as determined by the Lead Nurse Hospice at Home.

Person Specification Qualifications

GCSE or equivalent

Willingness to undertake relevant training

NVQ level 3 in health care

Skills and Knowledge

Excellent interpersonal and communication skills both verbal and written

Basic understanding of Hospice philosophy

Good understanding of holistic care

Examples of extended practice / skill set

Understanding of palliative care philosophy

Experience

Ability to work as part of a team

Previous experience in healthcare in professional capacity

Experience in community care (community roles only)

Experience working in more than one type of care settings e.g. hospice, hospital, care home and within a patients home
