Job Summary:
The Community Based Waiver Service Coordinator, Duals Integrated Care is responsible for managing and coordinating services for individuals who require long‑term care support and are eligible for community‑based waiver programs. The role ensures members receive necessary services and supports to live independently in their communities while coordinating care across various healthcare and social service systems.
Essential Functions
Engage with members in a variety of community‑based settings to establish an effective care coordination relationship, considering cultural and linguistic needs.
Conduct comprehensive assessments to determine the needs of members eligible for community‑based waiver services.
Develop individualized service plans that outline necessary supports and services, ensuring alignment with each member’s preferences and goals.
Serve as the primary point of contact for members and families, coordinating care across multiple providers and services, including healthcare, social services, and community resources.

Education and Experience
Nursing degree from an accredited nursing program or Bachelor’s degree in a health‑care field, or equivalent years of relevant professional experience.
Minimum of one year paid clinical experience in home and community‑/', etc.]
Medicaid and/or Medicare managed‑care experience is preferred.
Competencies, Knowledge, and Skills
Intermediate proficiency with Microsoft Office (Outlook, Word, Excel).
Prior experience in care coordination, case management, or working with dual‑eligible populations is highly beneficial.

Licensure and Certification
Current and unrestricted license as a Registered Nurse (RN), Licensed Social Worker (LSW), or Licensed Independent Social Worker (LISW) in the assigned state.
Case Management Certification is highly preferred.
Valid driver’s license, vehicle, and verifiable insurance. Employment is conditional on successful driver‑license record check and insurance verification.
Conditional employment pending successful criminal background check.
Influenza vaccination required during the influenza season (Oct. 1 – Mar. 31). Employees hired during that season have 30 days to complete vaccination and documentation.

Working Conditions
This is a mobile position; regular travel to homes, offices, and other public settings is essential.
Manually toil under varying weather conditions; may stand and/or sit for long periods.

Travel may exceed 50 % of time spent on duties.
Use of general office equipment (telephone, photocopier, fax, PC) required.
Flexible hours, including evenings and weekends, may be needed to serve members.
ន Compensation Range:
$62,700.00 – $100,400.00 (salary); base compensation may qualify for a performance‑linked bonus. CareSource offers a comprehensive total‑rewards package.
