Summary Objective The purpose of this position is to counsel and assess the needs of clients and families experiencing complex socio‑economic problems or severe family dysfunction. Assist clients in developing social service plans to achieve goals and objectives. Responsibilities may vary by assignment.

Essential Functions

Assess the needs of clients to determine and develop goals, objectives, and social service plans.

Identify indicators of progress and barriers to achieving self‑sufficiency and stability.

Provide counseling to clients experiencing multiple or complex socio‑economic problems, severe family dysfunction, or threat of loss of government assistance or housing.

Coordinate referrals, services, and activities with public schools, community ministries, government agencies, court system, and other social service providers.

Monitor and document client progress toward achievement of social service plans.

OSS Neighborhood Place

Assess the needs of clients and families.

Work with clients to develop service plans that include measurable, achievable, and timely goals.

Counsel and educate clients on topics relevant to their goals.

Provide referrals and actively connect clients with resources in the community.

Process applications for financial assistance.

Work with clients to develop discharge plans that include ongoing steps to improve stability.

Collect survey data from clients for quality assurance.

Assist Neighborhood Place staff and partners in planning and implementing programs, projects, and events that meet community needs.

Attend meetings and events to disseminate information and promote available programs and services.

Complete all documentation accurately and timely.

OSS Housing & Support

Assess the needs of formerly unhoused clients.

Assist clients with finding and securing housing.

Work with clients to develop service plans that include measurable, achievable, and timely goals.

Counsel and educate clients on topics relevant to their goals.

Provide referrals and actively connect clients with resources in the community.

Process requests for financial and material assistance as needed.

Conduct home visits as required by the funding source.

Help clients resolve situations that could lead to eviction.

Work with clients to develop discharge plans that include long‑term, stable housing.

Collect survey data from clients for quality assurance.

Assist Housing & Support staff and partners in planning and implementing programs, projects, and events that meet clients’ needs.

Complete all required documentation accurately and timely.

Minimum Education and Experience Requirements Bachelor’s Degree in Social Work, Psychology, Sociology, or related discipline.

Two (2) years of social service experience or an equivalent combination of education, training, and experience.

OR

Bachelor’s degree and four (4) years of social service experience.

High School Diploma or GED and six (6) years of relevant social service experience.

Other Minimum Requirements – General

Must provide one’s own source of transportation.

Must possess and maintain a valid state driver’s license with an acceptable driving history.

Other Minimum Requirements – OSS Housing & Support

Must provide community‑based case management and transportation services to clients as needed. Services include transporting clients to appointments to secure housing and access community resources. Home visits must be conducted with clients at regular intervals.

Physical Demands

Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.
