Scattered Site Supportive Housing Specialist Job Description
Established in 1948, The Mission of ASCEND Mental Wellness is to improve the quality of life for those affected by mental illness and to promote the awareness and importance of mental well-being in the community.
ASCEND Mental Wellness is seeking a Scattered Site Supported Housing Specialist to help and assist individuals in maintaining safe and affordable housing in the community.
The Supported Housing Specialist will visit clients at their homes throughout Warren and Washington counties.
Individuals must be well motivated and committed to making a difference in people's lives. Good listening, writing and verbal skills are required. The ideal candidate will be comfortable working independently.
Full-time positions offer a comprehensive benefit package including health and dental insurance, vacation, sick/personal time, holidays, floating holidays, and 401(k).
There is a sign-on bonus for this position of $1,000.
Scattered-Site Supportive Housing Specialist Education & Qualifications
High School Diploma, or equivalent, with at least two years' experience working in direct service provision in the human services field.
Valid NYS driver's license acceptable to Agency insurance standards.
Must be of good character and background and display an affinity for working with people with mental illness and/or co-occurring disorders.
Must have the ability to work independently, have good problem-solving skills and have the ability to work as part of a multi-disciplinary team.
Scattered-Site Supportive Housing Specialist Job Responsibilities
Participate in implementing program policies, referral and discharge procedures and follow up procedures.
Work with the client's families as appropriate.
Assist clients with community integration as warranted (i.e.- programs, services, entertainment, socialization, etc.).
Provide informal case management services and advocacy to clients as needed.
Maintain records/files in accordance to agency standards.
Attend regularly scheduled staff meetings and required in-service training.
Facilitate discharge/transition of clients and provide follow-up services to them.
Provide behavioral intervention when necessary and work with clients to utilize self-control/improved coping skills in de-escalation.
Assist with client moves.
Carry a client caseload of 20+
Represent the agency in a positive manner.
Assist in certification/re-certification processes as needed.
Complete special projects assigned by supervisor.
Maintain a safe environment for all clients and staff.
Maintain effective communication with other staff members and supervisors.
Preserve human dignity and confidentiality of all clients and act in the best interest of those served through the Agency's programs and services.
Always assure quality of work.
Adhere to all laws and regulations applicable to the Agency's business.
Adhere to the Agency's Corporate Compliance Program and established policies and procedures.
Always positively represent the Agency within the community.
Have adequate computer skills, or more refined for specific job titles.
Actively participate in scheduled team/staff meetings as scheduled by supervisor or as indicated by the program's services.
Actively participate in Agency sponsored staff development and in-service educational programs as deemed necessary by supervisor, including mandatory training assigned per job title.
Should be compassionate and demonstrate a commitment to helping people with mental illness and/or co-occurring disorders.
