Service Coordinator

Role Summary The Service Coordinator serves as a primary point of contact for clients and families, managing client intake, service coordination, and ongoing support. This role is responsible for conducting consultations, coordinating care start-ups, supporting service adjustments, and responding to client and family needs to ensure continuity of care, satisfaction, and quality outcomes. The Service Coordinator works closely with the Client Care Coordinator and internal teams to communicate service changes, resolve concerns, and maintain accurate documentation, while supporting compliance with company policies and HIPAA standards and delivering compassionate, client-centered service to seniors and others needing caregiver assistance.

Key Responsibilities

Manage and maintain Referral Partner Network (RPN) accounts and relationships

Participate in local community outreach, networking, and events to promote Home Instead services

Deliver presentations and educational outreach to referral partners and community groups

Support social media posting and engagement aligned with marketing initiativesServe as a point of contact for clients and families, supporting intake, service coordination, and ongoing communication

Assist with care start-ups, service changes, and coordination with internal teams to ensure continuity of care

Respond to client and family inquiries with professionalism and compassion

Document marketing activities, client interactions, and contacts accurately in Salesforce/WellSky

Maintain organized digital records and reports in compliance with company policies and HIPAA standards

Represent the organization professionally and mission-driven in the community and with clients

Qualifications

Strong public speaking and presentation skills (required)

Strong computer skills (required)

Salesforce, WellsSky or CRM experience preferred

Social media familiarity

Reliable transportation and ability to travel locally

Strong communication, organization, and time-management skills

Schedule Full time with flexible hours. Local travel and community engagement required.
