Position Details: The Community Engagement Liaison is a crucial link between our healthcare services and the community. This position focuses on driving business development and managing various communication channels. Acting as the local market leader for B2B (business-to-business), B2C (business-to-consumer), and DTC (direct-to-consumer) efforts, the Liaison represents our organization to referral partners, patients, and caregivers. Key Responsibilities: Business Development & Outreach: Identify opportunities for educational in-services within the healthcare community and coordinate their execution. Build and nurture strong relationships with referral sources, including hospitals, physician offices, senior living communities, and other healthcare providers. Seek out and pursue new business opportunities to enhance patient enrollment across various service lines. Represent the organization at community events, health fairs, and professional networking engagements. Patient & Family Engagement: Educate patients, families, caregivers, and legal representatives on the benefits and services available through Hospice. Act as a compassionate, knowledgeable resource to guide families through the enrollment process. Communication & Coordination: Develop and maintain relationships with medical staff offices, hospitals, and nursing homes, serving as a consultant for hospice and end-of-life information. Facilitate smooth communication between referral sources and internal clinical teams. Ensure prompt follow-up on referrals while maintaining accurate documentation of outreach efforts. Market Leadership: Provide educational opportunities to physicians, physician office staff, hospital social workers, nursing home staff, and community agencies. Collect and analyze referral pattern data from all sources, including physicians, social workers, discharge planners, and nursing facilities. Maintain an understanding of the local healthcare market, competitor activity, consumer demands, and policies affecting outreach goals and patient access. Act as the ambassador for the organization within the local market, promoting brand awareness and trust. Monitor market trends and competitor activities to inform strategic planning. Skills and Experience Required: Proven experience in healthcare marketing, sales, or community outreach, preferably in home health, hospice, or related fields. Demonstrated ability to meet or exceed performance goals and key performance indicators (KPIs), including referral volume targets, patient admission rates, community engagement metrics, and timely follow-up standards. Strong interpersonal and communication skills to engage diverse audiences effectively. Knowledge of Medicare/Medicaid and other payer sources is advantageous. Understanding of hospice eligibility and levels of care is preferred. Familiarity with customer relationship management (CRM) systems for tracking outreach activities. Able to work both independently and collaboratively in a dynamic environment. Valid driver's license and reliable transportation are required. Preferred candidates will demonstrate a strong history of utilizing data to inform decisions, enhance outreach strategies, and contribute to overall business development objectives.
