Job Details:

• BC in FM
• EMR is EHR NexGen
• two years of experience within a healthcare setting and leadership experience required
• Access health status of patient, review diagnostic reports, ensure the PC team nursing staff follows up with patients
• Develop and implement plan of care
• Perform minor surgical procedures
• Participate in peer review, quality assurance review, meetings, team hundles, particiapte in outreach and off-site health care programs in the community
• Reports to CMO
• 160 Vacation hours PTO 40 hours CME 40 hours and 2500 allowance, medical dental and vision, life insurance, 401k with 4% match and malpractice insurance
