What You’ll Do
Develop, plan, implement, and evaluate social service programs in accordance with state and federal regulations
Identify and address residents’ social, emotional, and psychological needs
Coordinate services to support resident well‑being and successful care outcomes
Collaborate with interdisciplinary team members to ensure holistic resident care
Maintain compliance with nursing home regulations, including MDS procedures and Medicare/Medicaid guidelines
What You Bring
Licensed Social Worker preferred (not required)
Minimum of one year of supervised social work experience in a healthcare setting
Experience working with the psychosocial needs of the elderly, preferred
Knowledge of nursing home regulations and reimbursement guidelines
Strong written and verbal communication skills
Positive, dependable, and team‑focused attitude
Excellent attendance and professionalism
