An established acute care organization is seeking an accomplished

Nurse

Director of Care Management

to guide clinical operations that impact patient flow, care transitions, and outcomes across the inpatient setting. This is a senior leadership opportunity for a clinically grounded professional who blends operational rigor with a people-first leadership approach.

In this role, you will shape the direction and performance of case management and utilization review services while leading a multidisciplinary team that includes nursing, social work, and care coordination professionals. You will be responsible for building efficient systems, strengthening interdisciplinary collaboration, and ensuring patients move safely and appropriately through the continuum of care.

Core Focus Areas Direct oversight of utilization review, care coordination, and discharge planning functions Leadership and development of RN Case Managers, Social Workers, and Care Coordination staff Performance monitoring using data-driven metrics and continuous improvement methodologies Optimization of length-of-stay management and readmission reduction strategies Alignment of care coordination services with regulatory, clinical, and operational priorities Partnership with physicians, nursing leadership, and executive teams to support patient-centered outcomes

Required Credentials & Experience Active California Registered Nurse license

(required) Bachelor of Science in Nursing (BSN) Minimum

5 years of progressive experience in Case Management Demonstrated ability to analyze utilization data, trends, and performance indicators Working knowledge of clinical documentation and care coordination workflows Strong communication, leadership, and organizational skills
