The Social Service Director is responsible for assisting in the planning, organizing, developing, and directing the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines, regulations, our established policies and procedures, and as may be directed by the Administrator, to ensure that the medically related emotional and social needs of residents are met/maintained on an individual basis.
Job Responsibilities
Orients resident and family to nursing center?khelp them adjust to the new environment.
Develop a social history for each resident.
Develop a social assessment and care plan, identifying medically related social and emotional problems and needs, and setting realistic goals and specific actions.
Answer admission inquiries and provide tours to prospective family members/residents.
Be actively involved in pre‑admission and admissions processes.
Interview resident/family as necessary in a private setting.
Listen and act compassionately with residents/family members; must work independently and maintain job‑related responsibilities.
Prepare and complete social service documentation in a timely manner.
Ensure notes are informative, descriptive of services provided, and of residents’ responses.
Participate in resident care‑plan conferences and be an active member of the care‑plan team.
Assist in arranging transportation as needed.
Refer resident/family members to appropriate social service agencies when services are not provided on‑site.
Document residents’ behavioral and psychosocial status.
Conduct discharge‑planning conferences with the resident, family, and appropriate staff.
Evaluate the resident’s potential for the safest and most appropriate discharge plan.
Utilize available resources to ensure a smooth transition to the community upon discharge.
Receive complaints from residents and families, forwarding them to appropriate staff to ensure resolution.
Document complaints.
Follow facility policies and procedures.
Reinforce to residents and families the facility’s desire to deliver the highest quality of care.
Develop a community resource file and establish contact with new providers.
Identify and promote a supportive family or surrogate network.
Provide individual and group help for family and residents at all times of adjustment, crisis, or particular need.
Provide intense, frequent, andപുര short‑term therapeutic groups in areas of adjustment, remediation, and self‑help skills.
Establish and assure growth of a separate volunteer program designed to meet specific social service needs.
Work with administration and staff to enable a fully functioning and self‑determining family counseling program to flourish.
In the absence of an Admissions Director, answer community inquiries about admissions, bed availability, and fee schedules.
Distribute pre‑admission packets and assist with facility tours as needed.
Education and Qualifications
Minimum Bachelor’s Degree páfrom an accredited social work or related field.
Previous long‑term care experience desirable.
Past supervisory experience preferred.
Computer experience preferred.
