Plymouth Health Services is a leading provider of skilled nursing and rehabilitation. As part of the North Shore Healthcare family, we operate 70 centers throughout Wisconsin, Minnesota, Michigan, and North Dakota.

Responsibilities Overall clinical management of the center, quality of care, and customer service. Accountable for resident/patient care, state and federal survey compliance, employee relations, clinic plan implementation, and center core clinical programs. Acts as the center’s clinical representative in the community. Completes rounds of the entire center premises daily to ensure compliance with all policies, procedures, and regulations.
Requirements
Current State Registered Nurse (RN) License in good standing
Demonstrated nursing management experience in a long‑term care setting
