At Grace Healthcare Services, we're looking for a RN Hospice Liaison to join our Interdisciplinary Team (IDT) in a contracted hospital setting. This role is a unique blend of clinical excellence, business development, and compassionate care, offering a fulfilling opportunity to support patients and families through critical transitions in care.
What You'll Do:
Build and nurture strategic partnerships with new and existing referral sources to facilitate appropriate referrals. Identify and assess the psychosocial, spiritual, and physical needs of patients in the inpatient setting. Coordinate comprehensive care plans, ensuring seamless transitions to the appropriate level of care. Conduct pre-discharge hospital visits, offering support and guidance to patients and their families. What We Offer:
