Claremont Hacienda, a Generations Healthcare MHRC facility, is looking for a Director of Social Service to join our team.

Job Description:

The Social Services Director is responsible for planning, organizing, and directing the overall operation of the Social Services Department at the Mental Health Rehabilitation Center (MHRC). This includes support of admissions, case management, discharge planning, and court liaison responsibilities. The Social Services Director ensures that residents’ social, emotional, legal, and discharge needs are identified and met in accordance with all applicable regulations, facility policies, and residents’ rights. This role works closely with the interdisciplinary team (IDT), families, courts, and outside agencies to promote residents’ well-being, facilitate smooth transitions, and support individualized treatment and recovery goals.

Essential Functions and Responsibilities:

Coordinate and support the admissions process, ensuring eligibility, documentation, and placement criteria are met.

Conduct psychosocial assessments at admission and orient residents and families/support persons (if applicable) to the MHRC program.

Collaborate with referral sources, hospitals, courts, and community partners to support timely and appropriate admissions.

Ensure residents’ psychosocial needs are identified and addressed in care planning and daily services.

Support residents in exercising their rights, making informed choices, and participating in treatment planning.

Assist residents in adjusting to the MHRC environment and provide orientation upon admission.

Promote a positive, respectful, and supportive environment for residents and their families.

Provide case management services from admission through discharge.

Develop individualized service plans addressing treatment goals, court mandates, and community reintegration.

Coordinate discharge planning, including arranging community services, housing, and outpatient supports.

Communicate regularly with conservators, families, and community providers to support continuity of care.

Maintain working relationships with hospitals, outpatient providers, case managers, and other resources to ensure successful transitions.

Serve as the primary liaison with the court system for residents admitted under mandate.

Prepare and submit required reports, updates, and documentation to courts and other legal entities.

Maintain effective communication with probation officers, attorneys, and other legal representatives to ensure compliance with court orders.

Complete timely and accurate assessments, care plans, progress notes, and discharge summaries in accordance with Title 9 and facility policies.

Ensure compliance with regulatory requirements related to social services, resident rights, and care coordination.

Participate in audits, surveys, and quality assurance activities.

Collaborate with nursing, medical, and program staff to ensure residents’ social and emotional needs are integrated into care planning.

Attend and contribute to resident care conferences and treatment planning meetings.

Support crisis intervention and de-escalation efforts when needed.

Supervise and support assigned social services staff, ensuring quality and timeliness of services.

Provide training and orientation on resident rights, communication strategies, and discharge planning.

Conduct performance reviews and foster professional development of social services staff.

Knowledge, Skills, and Abilities:

Knowledge of psychosocial needs of individuals with mental health conditions.

Knowledge of resident rights, admissions processes, case management, discharge planning, and community resources.

Knowledge of court-mandated treatment processes and ability to prepare and present reports to legal entities.

Knowledge of electronic medical records systems; PointClick Care preferred.

Strong communication, interpersonal, and conflict-resolution skills.

Ability to build positive relationships with residents, families, staff, and outside agencies.

Organizational and time-management skills to balance multiple priorities.

Ability to maintain confidentiality and exercise sound judgment.

Proficiency with documentation and use of basic computer applications (e.g., Office365 or similar).

Demonstrates a commitment to professional growth and lifelong learning.

Education And Experience

Bachelor’s degree in social work, psychology, sociology, human services, or a related field preferred; equivalent experience may be considered.

Minimum of 2–3 years of experience in a healthcare, behavioral health, or long-term care setting.

Prior experience in social services or case management within a SNF, MHRC, or related environment strongly preferred.

Supervisory experience preferred.

Basic Life Support Certification required.

Experience working with adults with moderate to severe mental illness preferred.
