The Housing Specialist supports vulnerable individuals and families by assisting them in achieving immediate and long-term housing stability. This role serves as a liaison between members, housing agencies, landlords, brokers, and community partners to facilitate access to housing resources and programs. The Housing Specialist acts as a subject matter expert within a care management team and provides hands-on, field-based support to members with complex housing needs. Key Responsibilities Conduct phone and in-person outreach to individuals experiencing housing instability. Perform comprehensive housing assessments to identify needs and appropriate services. Assist with housing applications and required documentation. Coordinate and obtain supporting clinical documentation, including psychosocial and psychiatric evaluations. Prepare individuals for housing interviews and accompany them to housing appointments as needed. Engage and communicate with housing agencies, landlords, brokers, and community-based organizations. Advocate on behalf of individuals to facilitate housing placement and address barriers. Educate individuals and caregivers on the housing process, tenant rights, and responsibilities. Serve as the housing subject matter expert for the care management team. Participate in external stakeholder meetings to strengthen partnerships and referral networks. Assist with expanding and maintaining a network of housing and social service referrals. Research and identify potential funding opportunities related to safe, affordable, and accessible housing. Maintain accurate, timely, and comprehensive documentation across multiple systems. Comply with organizational policies, training requirements, and performance standards. Perform additional duties as assigned by management. Required Qualifications Experience in processing housing applications and related documentation. Significant experience working with individuals affected by homelessness, mental health conditions, substance use disorders, and housing instability. Strong written and verbal communication skills. Proficiency with computers and web-based systems. Working knowledge of tenant rights and responsibilities. Preferred Qualifications Bilingual in English and Spanish. Familiarity with local housing systems and community-based resources.
