Client Stabilization Specialist Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona, Oklahoma and the District of Columbia. Job Summary The Client Care Coordinator is a shelter or temporary lodging site‑based position whose primary role is managing a housing caseload and coordinating care for individuals and families who are experiencing homelessness. The Coordinator will provide coordination of care and support services to assist clients in ending their current episode of homelessness, ensuring connection to existing or new providers for complex substance use, mental health, housing and medical needs. The Coordinator serves as a bridge, coordinating and monitoring community‑based services and facilitating their delivery. The role requires data entry into the established system, generation of program reports, and completion of an end‑of‑shift report documenting care coordination for each client.

Skills / Requirements

High school diploma or GED required. Associate degree (or higher) in a field related to Behavioral Health is preferred.

Minimum of six months of recovery from substance use and/or mental health disorders preferred.

Minimum of six (6) months of paid related work experience that provides adequate exposure to both behavioral and medical crisis situations is required.

1–3 years of full‑time health‑care related work experience with exposure to behavioral and medical crisis situations is preferred.

Certified Peer Support Worker or Recovery Coach certification required to be obtained within 90 days of hire.

Valid identification required.

Ability to pass a criminal background check required.
