Overview
Capitol River Wellness & Rehabilitation is seeking a
Licensed Social Worker
to lead our Social Services department as
Social Services Director . This role is ideal for a compassionate leader who values resident advocacy, teamwork, and quality care in a skilled nursing environment.
Responsibilities
Lead and support the Social Services department
Advocate for resident rights, dignity, and emotional well-being
Complete and oversee psychosocial assessments, care plans, MDS, and discharge planning
Collaborate with nursing and interdisciplinary teams
Maintain timely, compliant documentation in PointClickCare
Support state and federal survey readiness
Build strong relationships with residents, families, and community partners
Supervise and support Social Services staff
Qualifications
Licensed Social Worker (LCSW, LMSW, or equivalent) - Required
Bachelor’s Degree in Social Work or related Human Services field - Required
1+ year supervisory social work experience (healthcare preferred)
Skilled Nursing Facility experience strongly preferred
Strong communication and leadership skills
