Description Compensation Range:

$80,000 - $85,000 yearly commensurate with experience

Overview:

The Director is responsible for the implementation, management, and administration of a program to promote HIV viral load suppression programs and objectives, including managing contracts related to these initiatives. This position works collaboratively with a care team that includes the medical provider, client navigators, outreach staff/peers, nurse coordinator, medical case manager, and additional support and administrative staff. The Director will also be responsible for providing supervision of program staff who work with clients seeking HIV treatment and related services, including the full continuum of Housing Works’ clinical and supportive services.

It reports to the Managing Director for Biomedical Prevention and Navigation Services. The Director is responsible for managing the clinical and administrative functions by providing daily program oversight including but not limited to staff development and supervision, marketing, outreach, quality assurance, and data analysis/entry. As a leadership position in the organization, the Director is expected to exemplify the Agency’s core values and competencies and help operationalize the strategic vision and mission.

Program components include managing a team responsible for targeted outreach and linkage to care, peer-delivered services, care coordination, building community partnerships to increase client engagement, data entry and reporting, and supporting clients on a path toward stable engagement in care and viral load suppression by utilizing a multidisciplinary care team approach in a primary care and community setting. Team members work with a complex population that includes people who use drugs, people experiencing housing instability/homelessness, people with mental health issues, and others experiencing the effects of social and structural determinants of health. In addition to primary care, this position will collaborate with other Housing Works programs such as Youth and Prevention Services, OASAS Substance Use Treatment, and OMH Mental Health Care.

Responsibilities:

Ensure effective day-to-day management of program and contract activities

Responsible for monitoring the service-based aspects of contracts along with delivery of direct services.

Monitor contract compliance, including but not limited to service projections, staffing records, and fiscal performance including OTPS spending

Lead audit/site visit preparation activities to ensure service model fidelity and that administrative and fiscal operations meet program standards

Provide direct services which include care coordination, treatment adherence counseling, and crisis intervention when needed.

Effectively manage contract budget with support from department leadership

Provide supervision to the department staff, including Program Manager, Supervisor, RAD Specialist, Peers, and other staff

Collaborate with key stakeholders at each site, including leadership clinic staff, program staff to ensure effective communication and service delivery

Monitor programs’ enrollment and discharge trends to ensure retention and quality. Also monitor staff scheduling to always ensure adequate program coverage, as well as program financial trends to ensure targets are met at the site level

Track and report on progress toward meeting contract deliverables

Conduct data analysis, manage program dashboard, and performance metrics

Lead QI activities related to process and clinical outcomes

Liaise with funding organizations and participate in activities such as: TA calls, learning collaboratives, and other program meetings as required.

Ensure high-quality client experience by ensuring all client care is coordinated across Housing Works programs and addressing client issues and barriers to care as necessary

Program goals and participation across internal Housing Works programming

Other functions that may be required with this position include:

Participate in Housing Works advocacy events

Requirements:

Advanced degree (MSW, MPH, MPA, or related field) preferred

Five years or more of clinical experience in a health, mental health and/or social service setting.

Experience operating a service-based contract a plus, with a minimum of four years of supervisory experience in a similar setting.

Experience working with homeless populations and active substance users.

Knowledge and experience with the dually diagnosed community of HIV/AIDS and mental illness.

Knowledge and experience with state and local policies of NYC affecting persons living with mental illness, HIV/AIDS.

Experience in program planning and program operations.

Experience in MS Excel, time management and accomplishing task deadlines preferred.

Ability to supervise, manager and provide leadership to all staff members and trainees

Previous experience providing services and exercising leadership in a culturally and linguistically diverse setting, with demonstrated success

Previous experience working with substance use population

Previous experience of building partnerships with community-based organizations preferred

Bilingual English/Spanish preferred, but not required
