full-time Social Services Director
to lead our Social Services department and support residents and families throughout their stay. Key Responsibilities
Oversee daily operations of the Social Services department
Complete psychosocial assessments and care plan documentation
Provide resident and family support, advocacy, and resource coordination
Lead discharge planning and coordinate transitions of care
Participate in care conferences and interdisciplinary team meetings
Ensure compliance with state and federal long-term care regulations
Requirements
LSW required
Prior experience in a skilled nursing/long-term care or healthcare setting required
Strong communication, documentation, and organizational skills
