
Physical Therapy Assistant (PTA) – Skilled Nursing Facility
Flagstar Rehab is staffing Physical Therapist Assistants for our Rehab Department in state‑of‑the‑art Skilled Nursing Facilities (SNFs) throughout New York City and surrounding areas.
PTAs will work with a diverse multidisciplinary team to perform treatments and provide patient/family education to improve patients’ physicality and cognition and positively impact their daily functioning.

Pay Hourly Rate:
$31–$34 W‑2, $35–$38 1099
Job Duties
Collaborate daily with PT supervisor and follow the established Plan of Care for all patients.
Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
Monitor and document patient progress using innovative EMR software.

Current New York State PTA license and registration.
EMR competence.
