Responsibilities
Manages the day-to-day healthcare operations of the community to ensure residents\' healthcare needs are met, with respect and dignity.
Supervises licensed nurses and other direct care staff; strengthens clinical processes within the community until a replacement is identified, then supports the new Health and Wellness Director.
Travels within the division as assigned to support management vacancy, start-up, vacations, etc.
Direct supervision of community-based licensed nursing staff (LPN/LVN, RN), Med Techs/CMAs (as required by state regulations), and/or Lead Resident Care Associates; may supervise Resident Care Coordinators and Supervisor, Resident Care.
Assigns and directs work, appraises performance, rewards and disciplines staff, and addresses complaints.
Provides training and monitoring of medication administration, including auditing medication administration records.
Provides ongoing education to resident care associates with in-services and situation-specific training.
Maintains resident charts and reviews documentation performed by care-giving staff.
Assesses health, functional, and psycho-social status; initiates individualized service plans; proactively manages care and services; evaluates effectiveness; and maximizes residents
ability to remain in their environment.
Communicates with families and associates about resident health needs and scheduled appointments.
Involved in pre-admission screening; ensures required documentation is completed prior to or upon admission; updates assessments as required by policy and Nurse Practice Act.
Performs on-site evaluations of residents and maintains contact with families to support continuity of care and return to Brookdale when appropriate.
Coordinates with other departments to ensure quality, proactive care; leads meetings relevant to resident care issues.
Ensures scheduling and follow-through for in-house ancillary services (podiatry, doctor, dental, psych visits, lab, X-ray, ambulance, etc.).
Supports quality improvement activities and safety initiatives to reduce risk; shares on-call and manager-on-duty responsibilities as required.

Qualifications Education and Experience Education as required to obtain state nursing license (LPN/LVN or RN). Nurse management, senior living, or post-acute care experience preferred. Basic typing skills and working knowledge of personal computing/software are required, preferably in a Microsoft Windows environment.
Certifications, Licenses, and Other Requirements LPN/LVN or RN license.
Physical Demands and Working Conditions
Standing, walking, sitting
Interaction with co-workers, residents or vendors
Occasional weekend, evening or night work for shift coverage
Use of hands, reach with arms, on-call basis
Possible exposure to communicable diseases, blood-borne pathogens, and various hazards
Ability to lift up to 50 pounds; travel occasionally
Vision and other senses required
Management/Decision Making Applies existing guidelines to make varied decisions within a department. Uses sound judgment and experience to solve moderately complex problems based on precedent and reasonableness.
Knowledge and Skills Extensive knowledge of a distinct skill or function; strong understanding of the organization and work environment; working knowledge of a functional discipline.
