The Community Outreach Manager is a key external-facing role responsible for generating patient referrals and building lasting relationships with referral sources in Southern Jersey. You will be the face of our agency, educating healthcare providers and community organizations about our services to drive growth and help more patients access exceptional home-based care. Key Responsibilities

Develop and execute a strategic sales plan to meet and exceed monthly/quarterly referral goals. Identify, prospect, and build strong relationships with key referral sources, including: Physicians, specialists, and physician groups Hospitals and hospital discharge planners/case managers Rehabilitation centers, skilled nursing facilities (SNFs), and long-term care facilities Assisted living and independent living communities Community organizations, senior centers, and social service agencies Conduct effective sales presentations and service educational meetings with potential referral partners. Maintain a high level of knowledge regarding our services, eligibility requirements, and the competitive landscape. Track all sales activities, referral outcomes, and relationship details in the company CRM system. Represent the agency at community events, health fairs, and networking functions. Collaborate closely with the clinical and intake teams to ensure a smooth referral-to-admission transition and address any source concerns promptly. Stay current on industry trends and changes in healthcare regulations affecting home health. Qualifications & Skills

Required: Proven track record in sales, business development, or outreach, preferably in healthcare (home health, hospice, DME, pharmaceutical, or medical sales). Exceptional interpersonal, communication, and presentation skills. Strong self-motivation, goal orientation, and ability to work autonomously. Excellent organizational and time-management skills. Valid driver’s license, reliable transportation, and ability to travel daily within the assigned service area. High school diploma or equivalent. Preferred: Existing relationships with healthcare providers in the service area. Understanding of home health regulations and the post-acute care continuum. What We Offer
