is seeking an experienced
Associate Director of Case Management
to support the leadership and daily operations of its Case Management department. This is a high-impact leadership role, ideal for a nurse leader passionate about care coordination, patient flow, and quality outcomes.
Position Overview Reporting to the Director of Case Management, the Associate Director plays a key role in overseeing
day-to-day operations , staff supervision, and performance management of nurse case managers and support staff. The role focuses on optimizing
length of stay, discharge planning, utilization management, and interdisciplinary collaboration , while ensuring safe, efficient, and patient-centered transitions of care.
Key Responsibilities Provide daily leadership and operational oversight of the Case Management department Supervise nurse case managers and support staff, ensuring regulatory and clinical standards are met Monitor and improve
throughput, length of stay, discharge efficiency, and readmission reduction Lead interdisciplinary rounds and collaborate with physicians, nursing, social work, and ancillary services Support staff development, education, and performance management initiatives Participate in quality improvement, utilization review, and hospital-wide committees Assist with departmental projects, policy development, and strategic initiatives Promote excellent communication, customer service, and patient-centered care delivery
Qualifications BSN required; MSN preferred Active NYS RN license CCM preferred Minimum
5 years of clinical experience
in case management, utilization review, or discharge planning At least
2 years of leadership or supervisory experience
in a healthcare setting preferred Strong knowledge of utilization management, care coordination, and regulatory requirements Excellent communication, analytical, and organizational skills
