Physical Therapist – Home Health (Per Diem / Per Visit) — CareCo Medical Home Health is seeking a licensed Physical Therapist to provide skilled in-home therapy services to patients in the community. This is an excellent opportunity for an experienced clinician who enjoys working independently while collaborating with a supportive interdisciplinary team.

We manage acute and chronic conditions, provide wound care, medication management, and comprehensive therapy services.
Position Summary The Physical Therapist plans and provides physical therapy services to patients in their homes in accordance with the Plan of Care and agency policies. This role may also include case management for patients requiring skilled therapy services only. Therapists work collaboratively with nurses and other disciplines to provide care, education, and support to patients and their caregivers under the supervision of the Clinical Supervisor.
Pay and Schedule Per diem / per visit pay structure. Clinicians are paid per completed visit and documentation. Flexible work schedule. Mileage paid between patient visits.
Key Responsibilities
Perform physical therapy evaluations, assessments, and skilled treatments in the home
Complete initial and comprehensive assessments, including OASIS, within required timeframes
Develop, implement, and update individualized Plans of Care
Document all visits, findings, and outcomes in a timely manner
Communicate changes in patient status to physicians, case managers, and the care team
Provide patient and caregiver education to promote recovery and safety
Participate in discharge planning and interdisciplinary care coordination
Supervise Physical Therapy Assistants in accordance with agency and regulatory guidelines
Promote patient safety and comply with all agency, state, and federal standards
Qualifications
Graduate of an accredited Physical Therapy program
Current Physical Therapist license in the state of practice (Connecticut)
Minimum of three years of healthcare experience
Home health and OASIS experience preferred
Strong communication and documentation skills
Ability to work independently with minimal supervision
Valid driver's license and reliable, insured vehicle
