This position oversees the day-to-day healthcare operations of the community, supports services for residents, assesses health, functional and psycho-social status, initiates individualized service plans, proactively manages care, evaluates effectiveness, and maximizes residents’ opportunities to remain in the assisted living environment. The Wellness Director oversees and promotes residents’ health and wellness through regular visits, resident assessments, and ongoing communication with the resident, family, and MorningStar care team. Manage staffing needs of the Assisted Living/Memory Care Community by interviewing, hiring, training, and supervising the Assisted Living Coordinator, Reflections Coordinator, and MCMs Manage health care of all residents, including dissemination of information to families, staff, physicians, and third-party providers Perform duties in accordance with established methods and standards Conduct on-site assessments of residents admitted to hospitals or skilled nursing facilities; maintain contact with families and discharge planners Complete pre-admission assessments for each resident and update every six months or as needed with change of condition Initiate resident negotiated service plans; provide resident and family instruction; seek input from resident and family when developing the plan Perform ongoing assessments of residents’ physical and psycho-social needs and coordinate with other departments to ensure proactive quality care Order medications and oversee residents’ medication schedule; audit medication records and cart twice weekly; ensure proper documentation for medication administration On-call duties for staffing and scheduling; work as MCM when other MCMs are unavailable; provide coverage in emergencies to ensure medication assistance Ensure all infection control guidelines are met Knowledge of Assisted Living regulations, medication administration regulations, and occurrence reporting Qualifications
Demonstration of core values: Love, Kindness, Honesty, Goodness, Fairness, Respect License/Certification: Current Nursing License (LPN or RN) and First Aid certification as required Education: Minimum of 1 year of experience working with the elderly in an assisted living, skilled nursing, or hospital environment preferred Continuing Education: As required by law and in-service education within the community Job Knowledge: Knowledge of procedures and techniques for administering simple treatments; understanding of standard care practices; familiarity with universal precautions; ability to develop and execute negotiated care plans Other: Ability to read and interpret safety rules and manuals; strong communication skills in English; basic typing and computer skills Team members scheduled for 24+ hours per week are eligible for benefits; families of those team members may also be eligible Equal Opportunity Employer.
