
Overview
The Outreach Specialist is responsible for identifying, engaging, and assessing individuals in the community who are experiencing substance use disorders and may require withdrawal management or other services. This role includes conducting community-based substance abuse assessments to determine the need for withdrawal management, residential or outpatient programming, facilitating rapid access to services, and supporting individuals through the initial stages of treatment entry using a trauma-informed, client-centered approach.
Key Responsibilities
Conduct proactive outreach to individuals in the community who are experiencing active substance use and may require withdrawal management or other services, including street-based outreach, shelters, hospitals, and other community settings
Perform substance abuse screenings and assessments in the community to determine withdrawal needs, level of care, withdrawal risk, and treatment readiness
Facilitate rapid placement into programming, including bed coordination, intake scheduling, and transportation support
Provide education on the withdrawal process, withdrawal expectations, and treatment options for continuing care
Build trust and rapport with individuals using trauma-informed and culturally responsive engagement
Collaborate closely with Abraxas treatment team members, emergency departments, and community partners to ensure timely access to care
Advocate for individuals to overcome barriers to admission such as insurance issues, lack of identification, transportation, or stigma
Maintain accurate documentation of outreach encounters, assessments, and referrals
Participate in community outreach events, crisis response efforts, and ongoing training related to substance use and withdrawal management
Qualifications
Bachelor’s degree in social work, psychology, public health, human services, or a related field and 2+ years of proven success in healthcare outreach/admissions or a current credential in substance use disorder treatment with lived experience. The credential can be either in alcohol or other drug use or peer support and valid for 2+ years in the state of employment.
Experience conducting substance use assessments and working with individuals requiring withdrawal management services
Strong interviewing, crisis intervention, and motivational interviewing skills
Ability to work independently in community-based and high-acuity environments
Valid driver’s license and ability to travel daily throughout territory
