Pay Range:
$38.00 - $41.00 per hour depending on experience and credentials The New Jersey Eastern Star Rehabilitation and Nursing is currently seeking a Director of Social Services. The New Jersey Eastern Star Rehabilitation and Nursing is a well-established and well respected five-star facility as rated by CMS. Located in Central New Jersey, 2 miles off Route 287 at Exit 13, this not-for-profit facility, established in 1957, provides services for short-term rehabilitation patients and long-term care residents. Responsibilities include, but are not limited to: Manage the daily operations of the social work department. Provide therapeutic interventions and social work services for patients and families to ensure integrated, continuous care. Assess patients and complete all SS initial assessments, psychosocial needs, create treatment goals, and complete care plan with appropriate interventions. Complete MDS (SS sections) in timely manner for sub-acute as well as LTC residents/patients. Interview patients, families/significant (when appropriate) to identify needs and provide support as well as necessary resources. Participate in multidisciplinary care plan meetings and offer psychosocial context input to ensure it is integrated into the resident’s care plan. Provide regular updates to insurance companies for Managed Medicare sub-acute patients. Serve as an advocate for all patients and families, including advanced care planning discussions. Offer psychosocial support during illness adjustment and life changes. Actively facilitate resident admissions, transfers, and discharges. Maintain accurate documentation per facility policy, state, and federal regs Build/maintain networks with community agencies and government services. Maintain list of residents on psychological services. Participate in QAPI program. Ensure departmental compliance with all regulations (state and federal) that pertain to social work services in long term care Requirements: Current NJ social work licensure/certification required. MSW from an accredited program preferred. Must have at least 2 years’ experience in long term care setting. Knowledge of Medicare, Managed Medicare, Medicaid programs, rule and regulations, documentation Strong communication, negotiation, and conflict resolution skills. Must pass background check, physical and drug testing. Flu shot is Mandatory. Given by facility.
