Position: Medical Documentation Specialist
Location: Roseville, MN
Hours: Monday-Friday 8am-5pm (In office)
The Position We are looking for a full time Medical Documentation Specialist to join our exceptional customer service team who is committed to helping our patients thrive. Our Customer Service department is a fast-paced, high-energy environment and you’ll find yourself challenged and required to use critical thinking every day through a variety of in-person, email and phone conversations. You will also be surrounded by a close-knit and supportive group who all have the same dedication to serving medically complex children and their families.
Ensures medical records are complete and accurate per regulatory and company standards
Input of medical records is completed in accordance with time standards
Audits records and prepares reports on inaccuracies
Confers with prescribers and other health personnel to assure complete, current, and accurate medical records
Researches records to locate health data as requested
Maintains and utilizes a variety of health record indexes and storage and retrieval systems
Operates computer to process, store, and retrieve health information
Assists in identification of medical records needed for research using both manual and computer indexes
Assists Customer Service Department in special studies or research
Presents an overview of medical records process to new staff
Prepares input for computerized medical information systems and for processing forms and documents
Processes Hospital Board to track patient admits to and discharges from the hospital
Updates patient records when notified of patient information changes
Maintains prescribers databases
Process approved release of information request
On-call duties/rotation may be required based on business need
Other duties as assigned
The Qualifications
Durable medical equipment (DME) intake/ admissions experience preferred
High school diploma or GED; or one to three months related experience and/or training; or equivalent combination of education and experience
Knowledge of Database software; Spreadsheet software and Word Processing software
Exceptionally organized, multi-tasking, problem solving and communication and strong keyboard skills. A high level of initiative combined with a customer service mentality is also required.
Experience in Brightree EHR a plus!

Our services allow our clients to remain and thrive in their homes, schools, and communities. The majority of the clients we serve are age’s birth through 21 years old and have a variety of technologies that might include a trach, ventilator or g-tube.
SALARY RANGE At PHS, we are committed to providing specialized care for medically complex children. We are dedicated to offering fair and transparent compensation to our team members. The range for this role is $18.43 to $20.27 hourly, based on experience and qualifications.
