Key Responsibilities
Build and maintain strong relationships with referral sources to consistently meet or exceed sales goals.
Advocate for patients, ensuring a safe and seamless transition from healthcare settings back to their homes.
Collab with interdisciplinary teams, attending weekly case conferences and coordinating with community healthcare teams.
Educate residents, caregivers, and wellness directors with ongoing clinical education.
Ensure continuity of care, communicating smoothly between physicians, community, and home care teams.
Oversee the referral‑to‑admission process, obtaining and processing all required documentation efficiently.
Maintain regular communication with the community executive team to ensure seamless resident care and continuity.
Qualifications
Clinical background preferred and previous healthcare sales experience required (e.g., skilled nursing facilities, DME, ortho, cardio).
Excellent interpersonal communication and presentation skills.
Proficiency in Microsoft Office.
Detail‑oriented and able to travel within the assigned territory and to sales meetings as required.
Exceptional customer service and written communication skills.
Valid driver’s license, auto insurance, and reliable transportation.
Home health care experience a plus.
Benefits & Compensation
Competitive base salary: $70,000 + robust bonus structure after meeting minimum episodic quota.
Flexible business hours with potential Monday–Sunday responsibilities; weekend availability may be required.
Three weeks PTO, increasing to four weeks after five years.
401(k) with employer match and annual increases.
