Description
Conduct thorough evaluations of patients in both inpatient and outpatient environments and craft personalized treatment plans
Deliver engaging physical therapy interventions targeting mobility, strength, balance, and overall function
Collaborate with fellow nurses, physicians, case managers, and rehabilitation experts to maximize patient outcomes
Guide patients and their families in understanding therapeutic objectives, discharge strategies, and at-home exercises
Document evaluations, treatment sessions, and progress meticulously, adhering to facility and regulatory standards
Participate in team rounds and interdisciplinary discussions to ensure seamless and coordinated care
Education
Doctorate in Physical Therapy (DPT) from an accredited institution or equivalent degree
Certifications
Valid state licensure as a Physical Therapist
CPR certification preferred
Additional specialized qualifications, such as in neurology or geriatrics, are beneficial but not essential
Skills
Prior experience in a physical therapy setting strongly preferred
Proven ability to assist patients with complex medical conditions and mobility challenges
Excellent communication and teamwork skills
Proficient in utilizing digital charting software and clinical documentation tools
