Social Worker Director

Responsibilities The Director of Social Work is responsible for the overall leadership and organization of the Social Work department. This position will provide administrative and clinical supervision to social workers and discharge planners to ensure quality of service to the long‑term care and rehabilitation units for Stella Maris. Services include admission, psychosocial assessment, plan of care, linkage with community resources, counseling, crisis intervention, and discharge planning.

This position will provide direct social work services to nursing home and rehabilitation patients and families as needed. The Director also ensures the departmental documentation is in compliance with State and federal regulations for the department.

Responsible for planning, developing, organizing, implementing, evaluating and directing the operations of social service programs for the facility.

Promotes the mission, vision, and values of Stella Maris

Develop, plan and implement program goals, objectives, personnel, resources and equipment.

Develop and implement policies and procedures for identifying the medically‑related social and emotional needs of the patients/residents.

Assists in providing solutions to social and practical environmental problems, including seeking financial assistance, discharge planning (including collaboration with community agencies) and providing referrals to other community agencies when specialized assistance is required.

Provides supervision to staff that meets regulatory compliance.

Provide clinical consultation, oversight of case planning and care coordination.

Responsible for a caseload.

Direct crisis intervention efforts and intervene as required.

Provide staff management to include hiring, development, training, performance management and communication to ensure effective and efficient department operation.

Ensure procedures and workflow are consistent with organizational objectives, treatment protocols, regulations and standards of care.

Ensures regulatory compliance with MDS, BIMS, PHQ9, Care planning.

Qualifications

Licensed SW in the State of Maryland required

Degree in SW required

Minimum of 3 years LTC experience required

Prior supervisor experience preferred

Minimum: USD $33.90/Hr. Maximum: USD $50.80/Hr.
