Job Summary

Position title:

Director of Integration, Care Management

Reporting to:

Senior Vice President, Operations

The Director of Integration, Care Management will lead the planning and execution of integration initiatives across Essen Health Care's Care Management Division. This role is responsible for managing complex projects that bring together clinical operations, partnerships, technology, and workflows to ensure successful onboarding of new partners, programs, and service lines.

The ideal candidate is a highly organized healthcare leader with strong project management experience, operational insight, and the ability to drive cross-functional collaboration across clinical, IT, compliance, finance, and operations teams.

Responsibilities

Integration & Project Management

• Lead end-to-end integration projects for new Care Management partners, programs, and operational initiatives

• Develop detailed project plans, timelines, milestones, and deliverables

• Ensure successful implementation of workflows, staffing models, systems, and reporting structures

• Identify risks, dependencies, and mitigation strategies to keep projects on track

• Serve as the central point of coordination between internal departments and external partners

Operational Execution

• Translate strategic goals into executable operational plans

• Standardize onboarding and integration processes for new partners and care management programs

• Ensure alignment between clinical operations, compliance, and performance expectations

• Monitor post-integration performance and address operational gaps

Cross-Functional Collaboration

• Partner with Clinical Leadership, IT, Compliance, Finance, HR, and Operations to ensure seamless integration

• Facilitate regular project meetings, updates, and stakeholder communications

• Drive accountability across workstreams and ensure deadlines are met

Process Improvement & Optimization

• Develop scalable integration frameworks and best practices

• Continuously improve workflows for care management delivery and partner engagement

• Leverage data and performance metrics to inform decision-making

Reporting & Governance

• Provide regular project status reports to executive leadership

• Track KPIs related to integration success, operational readiness, and partner performance

• Support regulatory and compliance readiness for all integration initiatives

Qualifications

Required Qualifications

• 5-7+ years of experience in healthcare operations, project management, or care management integration

• Proven experience leading complex, cross-functional projects in healthcare environments

• Strong understanding of care management, population health, or value-based care models

• Experience working with Medicaid managed care and regulatory frameworks preferred

• Excellent organizational, communication, and stakeholder management skills

• Bachelor's degree required; Master's degree (MBA, MHA, MPH) preferred

• PMP or formal project management training strongly preferred

Preferred Qualifications

• Experience integrating new healthcare partners or service lines

• Background in scaling healthcare operations across multiple sites

• Familiarity with clinical workflows, EHRs, and care management platforms

• Knowledge of NYC/Bronx healthcare market
