Casa Esperanza is seeking an enthusiastic and skilled Reentry Outreach Specialist to support individuals returning from incarceration within the Reentry Program. The program aims to reduce relapse and recidivism among Latine reentering citizens living with co-occurring substance use and mental health disorders by expanding access to integrated, bilingual/bicultural services that prevent homelessness, unemployment, and relapse. The Reentry Outreach Specialist will connect with correctional facilities to assist in the transition from corrections to a recovery-oriented continuum of care and will educate justice-involved individuals about community-based addiction, mental health, employment, and housing services within Casa Esperanza’s continuum and across our provider network. The role includes completing Level of Service/Case Management Inventory (LS/CMI) assessments with currently incarcerated individuals to identify reentry and behavioral health treatment needs, designing a reentry plan that supports sustained recovery and access to essential services such as health insurance, housing, and employment. The Reentry Outreach Specialist coaches clients, promotes engagement, and facilitates community participation for current inmates and reentering citizens. They will advocate for clients at court, promote Casa’s continuum of care to the justice system, and collaborate with Community Corrections, Courts, and Law Enforcement to support successful reentry. The Specialist will help recruit justice-involved individuals for participation in Casa’s PAR (Programa de Pares Apoyando Recuperacion) curriculum in partnership with the Clinical/Curriculum Coach, support program benchmarks, timelines, goals, and objectives, and report to payers and regulators as required. As part of the Casa Esperanza team, you will provide services in a fully integrated, holistic, and collaborative manner using a team-treatment approach to coordinate all client services across Casa programs and external providers. This model emphasizes clear communication, reduces duplication of efforts, and ensures wraparound supports to maximize treatment effectiveness and outcomes. What You’ll Do

Service Delivery Work with the Project Manager and Recovery Support Services staff to implement outreach and community mapping strategies to identify and reach Latines at high risk for substance use, criminal justice involvement, and recidivism Develop, schedule, and facilitate support groups and reentry clinics, virtually and in-person Participate in Reentry Panels to provide education and resources to incarcerated individuals Conduct assertive outreach to correctional facilities, specialty courts, parole/probation boards, and other organizations serving returning citizens Conduct LS/CMI assessments with incarcerated individuals to identify strengths, needs, abilities, and preferences Provide health insurance and benefits counseling, and reentry planning in preparation for release Use Motivational Interviewing and Stages of Change to support engagement and motivation Engage or reengage clients in behavioral health services, including individual, group, and family therapy; psychiatry and medication-assisted treatment; community-based case management; and peer/self-help supports Ensure referrals to primary care, dental, financial, legal, education, training, and employment services Provide transportation assistance to and from correctional, treatment, and support services Conduct regular outreach to returning citizens who have disengaged from behavioral health services Provide advocacy support during court appointments and hearings Provide ongoing health promotion regarding substance use, mental health, STD/STIs, HIV, HCV, nutrition, exercise, and chronic condition management Represent the agency with community education and advocacy Maintain ongoing engagement to locate participants for grant follow-up interviews and reengagement as appropriate Collaborate with Marketing and Communications to keep an up-to-date list of community partners Develop and maintain outreach materials to educate patients, families, and partners about Casa services and policy Other duties as assigned Cross Departmental Collaboration Build strategic community partnerships with Marketing, Communications, and leadership to support a multi-agency approach to criminal justice reform and reentry services Coordinate with the Patient Access team to inform new participants about reentry services Collaborate with Casa’s Community Support Program for ongoing case management, psychoeducational groups, and care coordination Engage with the Multidisciplinary Team to retain clients in care Help clients engage in prosocial activities and a recovery-focused community Administrative Document all client encounters and maintain up-to-date records in the electronic health record, including LS/CMI assessments Meet data collection requirements within specified timeframes Attend bi-weekly supervision and relevant case conferences and staff meetings Support reporting for program funding sources Facilitate Recovery Support Services groups and document group notes in the electronic health record Other duties as assigned Qualifications

Bilingual Spanish/English is required Ability to obtain clearance for, and willingness to travel to, correctional facilities Experience outreaching to individuals at risk for SUD, addiction, mental illness, trauma, violence, and/or those involved in the criminal justice system Ability to work weekends and evenings as needed Familiarity with Latine health, social, and cultural issues Ability to work as a team member and independently Understanding of harm reduction techniques Strong verbal and written communication skills Strong interpersonal and crisis management skills Minimum associate degree in a related field Computer skills (Microsoft Word or equivalent) Valid Massachusetts Driver’s License Must be or willing to become CAC certified within 30 days of employment Must obtain Human Subjects Certification within 30 days of employment Schedule

Full time Availability to work 8am–8pm, Monday–Friday, and weekends as needed What We Offer

22.28 – 22.95 USD per hour (Familias Unidas Outpatient Program)
