Responsibilities

As a Director of Social Services, you will lead our Social Services Department and the programs within it. Your work will play a crucial role in ensuring the emotional, social, and psychological well-being of our residents.

Develop and implement comprehensive social services programs that address the diverse needs of our residents

Provide leadership to the Social Services team, ensuring the delivery of high-quality, person-centered care

Collaborate with interdisciplinary teams to create individualized care plans that enhance the overall well-being of residents

Act as a liaison between residents, families, and community resources to support a seamless transition into our facility and assist with discharge planning

Advocate for residents' rights, ensuring they receive the highest standard of care and maintaining their dignity and independence

Qualifications

Master’s degree in social work or related field

Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) preferred

Minimum of 4 years in social services, with at least 2 of those years in a leadership role in a long-term care setting

Strong understanding of the aging process, psychosocial needs of the elderly, and the impact of chronic illness

Ability to collaborate effectively with a diverse team and build positive relationships with residents and their families
