JOB DESCRIPTION – THE COMMUNITY AND OUTREACH SPECIALIST WILL PLAY A VITAL ROLE IN CONNECTING COMMUNITY MEMBERS TO FREE COLORECTAL CANCER SCREENINGS AND OTHER ESSENTIAL HEALTH SERVICES. THIS POSITION IS RESPONSIBLE FOR PROMOTING AWARENESS OF AVAILABLE PROGRAMS THROUGH GRASSROOTS OUTREACH, HEALTH FAIRS, COMMUNITY EVENTS, AND SOCIAL MEDIA. THE SPECIALIST WILL COLLABORATE WITH LOCAL ORGANIZATIONS, HEALTHCARE PROVIDERS, AND COMMUNITY LEADERS TO REDUCE BARRIERS TO CARE, EDUCATE THE PUBLIC ABOUT SCREENING OPTIONS, AND FACILITATE ACCESS TO FREE SERVICES. KEY DUTIES INCLUDE ORGANIZING OUTREACH ACTIVITIES, BUILDING RELATIONSHIPS WITH STAKEHOLDERS, PROVIDING INFORMATION AND NAVIGATION SUPPORT TO PARTICIPANTS, AND TRACKING ENGAGEMENT TO ENSURE PROGRAM EFFECTIVENESS. THE IDEAL CANDIDATE IS PASSIONATE ABOUT HEALTH EQUITY, HAS STRONG COMMUNICATION SKILLS, AND IS COMMITTED TO IMPROVING COMMUNITY HEALTH OUTCOMES.
Roles And Responsibilities
Build and maintain partnerships with local clinics, hospitals, and community organizations to coordinate screening days, distribute stool-based test kits, and streamline referrals into care. Establishes and maintain active and frequent contact with local community organizations and employers (Greater Philadelphia Region).
Lead grassroots outreach across health fairs, barbershops/beauty salons, faith‑based sites, community centers, and social media to promote free CRC screenings and related services. Establishes rapport with area businesses.
Provide patient navigation—educate residents on screening options, assist with appointment scheduling, request physician orders for stool tests when appropriate, and follow up to ensure test completion. Assist employers in recruiting, interviewing, and hiring qualified applicants.
Reduce barriers to care by arranging reminders, coordinating with providers, and helping residents overcome access challenges that could prevent screening and follow‑up. Keeps project staff appraising of local businesses needs and job placement opportunities.
Cultivate trusted relationships with community leaders, healthcare providers, and stakeholders to increase awareness and confidence in free services.
Plan and staff outreach activities, track participation and conversions (e.g., registrations, completed tests), and document engagement to evaluate program effectiveness. Enlists cooperation of community and business leaders to help increase awareness of available workforce services in the community.
Coordinate closely with the CRC Care Navigator/program team, meeting regularly to align targets, share data, and address gaps in completion. Arranges for employers to speak to job seekers and employer expectations.
Represent the program in partner and funder meetings and prepare concise progress updates and recommendations for leadership.
Other administrative duties as assigned.
Requirements
Demonstrate customer service and/or sales experience.
Excellent verbal and written communication skills.
Familiarity with the communities being served, with knowledge and understanding of local needs and resources.
Demonstrated knowledge of labor market, trends (Greater Philadelphia Region).
Public speaking experience.
Demonstrated computer skills, with experience using MS Word, Excel, PowerPoint, and Outlook.
