We are actively seeking candidates with a background in social or human services to become IRIS Consultants. This remote position involves building strong partnerships with individuals in your community enrolled in the Wisconsin IRIS Program and the TMG IRIS Consultant Agency. Although you'll work from home, you will regularly visit IRIS participants in their homes and communities.
As an IRIS Consultant (IC), you will guide participants through the Wisconsin IRIS program, helping them set and achieve their long-term care goals. You will play a vital role in helping individuals from diverse backgrounds and abilities live the lives they choose. Our participants often express gratitude for the support and positive impact provided by our ICs!
Within this role, you will connect participants to essential community resources and assist them in developing personalized IRIS plans focusing on areas such as employment, housing, health, safety, community involvement, transportation, and building lasting relationships. No two days will be the same, providing you with a diverse and engaging work experience!

KEY RESPONSIBILITIES
Conduct a minimum of four in-person meetings each year with IRIS participants, including one required home visit annually.
Provide program orientation for new participants, covering their rights, responsibilities, and necessary documentation.
Gain a comprehensive understanding of the participant's life, including goals, relationships, and community connections.
Assist participants in identifying personal outcomes and ensure ongoing support while adhering to their IRIS budget and program regulations.
Document all planning activities and updates using the IRIS data system (WISITs) within 48 business hours.
Research and share relevant community resources that align with individual participant goals.
Regularly track and document participant progress and changes whenever adjustments are needed.
Assist participants with budget management, including interpreting reports and liaising with Fiscal Employer Agencies.
Foster trusting, engaged relationships while effectively communicating program updates and compliance issues.
Ensure confidentiality and HIPAA compliance in all interactions.
Collaborate with colleagues to successfully implement participant plans.
Attend monthly team meetings and participate in scheduled phone check-ins with a supervisor.
QUALIFICATIONS
To qualify for this role, you should possess:
At least 2 years of experience in health care, ideally in care coordination, with at least 1 year serving the IRIS program's target groups (adults with disabilities or older adults), or a combination of relevant education and experience.
A bachelor's degree in social work, psychology, human services, counseling, nursing, special education, or a related field (or four years of equivalent experience).
A valid driver's license, reliable transportation, and adequate auto insurance for work-related travel.
Strong relationship-building skills with coworkers, clients, participants, providers, and agencies.
The ability to work independently and stay self-motivated.
Knowledge of long-term care programs and self-determination principles.
Effective problem-solving and critical-thinking abilities.
Excellent time-management and prioritization skills.
Capacity to multi-task, adapt to change, and maintain professionalism in diverse situations.
Proficient knowledge of community resources.
Detail-oriented with a proactive approach.
Outstanding verbal and written communication skills.
Proficiency in Microsoft Office suite and related software.
Compensation : Pay ranges from $19.84 to $38.69 per hour. Actual compensation may vary based on geographic location, experience, education, and skills. TMG offers a competitive benefits package and is an Equal Opportunity Employer (EOE) M/F/D/V.
