Title: Patient Growth Specialist (Outreach Executive I) Company: Oak Street Health Location: 3536 W Glendale Ave, Phoenix, AZ 85051 Role Description: The Patient Growth Specialist (internally known as an Outreach Executive) is a community-facing role dedicated to connecting Medicare-eligible seniors with Oak Street Health. As the primary lead generators for their territories, Outreach Executives act as brand ambassadors, sharing the impact of our unique care model to drive local patient growth. Working in close collaboration with Outreach Managers, Directors, and Community Relationship Managers, you will execute targeted strategies to build trust and expand our patient base within your assigned market. Core Responsibilities
Sales & Lead Generation Generate high-quality leads by executing community events and conducting localized outreach within the Medicare senior community. Identify and convert leads through cold calling, digital channels, and referrals from the broader outreach team. Persistently engage prospective patients via email, phone, and face-to-face networking to build a robust sales pipeline. Stay current on service updates, marketing initiatives, and organizational workstreams through active participation in team briefings. Partner with Community Relationship Managers (CRMs) and Outreach leadership to align on lead generation strategies and Insurance Agent (IA) partnerships. Relationship Management Meticulously document all customer interactions and sales activities within the CRM and internal business systems. Serve as the primary liaison between clinical teams and new patients to ensure a seamless transition into our care. Address and resolve prospect concerns or complaints with empathy, ensuring a positive first impression of Oak Street Health. Maintain consistent touchpoints with prospects to foster trust and long-term community presence. What We're Looking For
Required: 2+ years in Sales or Healthcare environments. Must be fluent in English and Spanish languages to connect with our local patient population. Proven ability to cold call, manage digital lead channels, and influence key stakeholders. Exceptional verbal and written communication skills with a professional phone presence. Skilled at navigating external barriers and managing multiple priorities in a fast-paced environment. Must have a valid driver's license and reliable transportation. Note: This role is 60% field-based and 40% clinic-based. Flexibility to work weekends for community events and patient engagement. US work authorization Preferred: Strong computer skills, including basic knowledge of Microsoft and/or Google Suite Experience with Customer Relationship Management (CRM) and/or Electronic Medical Record (EMR) software is a plus. Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $18.50 - $35.29 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
