Independent Optometrist – Walmart

Location: 4425 Venetucci Boulevard, Fountain, CO , United States of America.

Join us at Eyes On Eyecare to lead eye and vision care as an independent optometrist leasing space inside our Walmart stores.

Salary: Based on experience.

Schedule: Full-time.

Position Summary

As an independent lease holder, you will take a leading role in patient care with respect to eye and vision care as well as general health and well‑being. You will control your practice, including:

• Days of practice
• Hours of operation
• Patient schedule
• Fee schedule (keep 100% of exam/professional fees)

Equipment and Support Options

• Equipment/instrumentation
• Administrative and/or Opt-Tech support services (if state allows)

Minimum Qualifications for Lease Agreement

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9
• ME & VA license agreements are with Gumberg Asset Management Company
