What Youll Be Doing:

•  Providing one-on-one care to patients in their homes no double booking, no packed gym floors
•  Building and carrying out individualized treatment plans focused on real-life function
•  Working closely with your mentor and interdisciplinary team
•  Learning home care best practices while sharpening clinical judgment
•  Documenting visits in a user-friendly EMR (we promise, it wont ruin your evenings)
