Social Services Director We are seeking a dedicated Social Service Director to lead our team. Social Services is that integral part of our interdisciplinary team. Working with therapy, nursing and the rest of the team to ensure the quality of the residents stay followed by a smooth transition back into their home life. The Social Services Director is responsible for monitoring the residents\' mental and psycho-social needs and to provide the services to meet these needs in order to attain or maintain the highest practical level of physical, mental, and psycho-social well-being.
Responsibilities
Develop an ongoing care plan based on the residents’ needs, interests, and abilities as identified in the initial assessment.
Participate in care plan meetings.
Complete required record keeping upon resident admission, discharge, transfer; chart and document per facility policy.
Implement healthcare directives & identify end of life care opportunities.
Assess the resident\'s potential for discharge as a member of an interdisciplinary team.
Plans for the resident\'s discharge and post-discharge with the resident, family, and interdisciplinary team.
Assist the resident and family in the implementation of the discharge plan.
Qualifications
Bachelors degree in Social Work
2-3 years’ experience in a social services/discharge planning in a long-term care/Skilled nursing setting
Working knowledge of Medicare/Medicaid & Insurance
