Position: Independent Optometrist leasing space inside Sam’s Club.

Employer: Eyes On Eyecare.

Salary: Based on experience. Full-time position.

Location: 301 Coit Rd, Plano, TX .

Key Responsibilities:

• Establish days of practice and hours of operation.
• Control patient schedule and set fee schedule.
• Maintain 100% of exam/professional fees.

Fair Market Value Options (optional):

• Advanced equipment/instrumentation.
• Administrative and/or opt-tech support services if state allows.

Minimum Qualifications for Lease Agreement

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist MD or DO from an accredited school.
• Valid state ophthalmology license.
• General liability certificate / professional liability insurance certificate.

Additional Information: Lease agreements are handled by Gumberg Asset Management Company.
