Position Summary
We have an exciting opportunity to join our team as an RN 4 North Med/Surg Tele 7a-7:30p.
Schedule
We offer a flexible schedule of 12 (12-hour) shifts per month with bi-weekly weekend rotations.
Responsibilities
The registered nurse collects patient health data by involving the patient, family and other health care providers. Performs initial assessment based on the patient’s presentation. Records relevant data on the nursing component of the Interdisciplinary Admission Assessment. Demonstrates awareness of the special needs of each age group or culture served as well as those patients with disabilities. Performs ongoing assessments as established by department policy and as warranted by the patient’s response. Incorporates discharge planning into the admission process. Analyzes assessment data to identify problems and determine expected outcomes individualized to the patient. Demonstrates the ability to combine theoretical knowledge and clinical expertise to anticipate and plan for identified patient and family care needs. Identifies actual or potential problems with validation from the patient, family and other health care providers. Identifies cultural, ethnic, and spiritual aspects of patient/family dynamics related to care. Recognizes comfort/pain management needs impacting the plan of care and outcomes. Communicates expected outcomes to the patient, family and other health care providers to promote continuity of care. Develops and implements a plan of care with interventions to attain expected outcomes; collaborates with patient, family and appropriate healthcare providers. Formulates a family-centered plan of care aligned with physician treatment plan and integrates cultural aspects of care. Incorporates teaching and learning principles based on identified learning needs and developmental level. Assists the patient and family in identifying and securing services to address health-related needs. Maintains documentation of the plan of care and updates as required; sets realistic goals and priorities; uses time efficiently. Implements the plan of care and monitors the patient; adapts nursing procedures to the patient’s needs. Provides education to the patient and family; recognizes hazards to safety and maintains a safe environment. Works collaboratively with other team members to implement interventions and delegates tasks as appropriate per state nurse practice act. Demonstrates knowledge of pharmacology, including drug administration rules and safety considerations; follows medication policy and documentation procedures. Evaluates progress toward outcomes and documents patient responses and revisions to the plan of care. Identifies barriers to achieving outcomes and uses resources to address them; communicates with patient, family and other providers to promote continuity of care. Demonstrates ethical practice and respect for patient rights; supports customer satisfaction and participates in quality improvement of nursing practices. Maintains professional interactions within the unit and with other departments; participates in professional development and uses resources to improve patient care. Engages in performance appraisal and collaborates with management to improve the environment of care for patients, families and personnel. Minimum Qualifications
Professional knowledge of nursing theory and practice from completion of an accredited School of Nursing (two-year training). Current New York State license or permit as a Registered Professional Nurse. Interpersonal skills to effectively interact with patients and families, with cultural sensitivity. Customer service orientation; ability to communicate clearly in verbal and written format. Analytical ability to assess and develop solutions to clinical situations or unit problems. Maintains knowledge and proficiency in the clinical area and age group served. BLS certification (AHA) and de-escalation training; ACLS (ICU/Float pool). Ability to frequently lift/carry more than 75 lbs; ability to frequently push/pull more than 100 lbs; ability to frequently bend, squat, kneel, climb steps/ladder and reach above shoulders.
