As our
Social Services Director , you will play a critical leadership role, advocating for residents, supporting families, and ensuring that every individual receives compassionate, person-centered care in accordance with federal and state regulations.
Position Overview The
Social Services Director
serves as the facility’s primary liaison for residents’ social interests and needs. Under the direction of the Administrator, this role plans, organizes, develops, and directs the overall operation of the Social Services Department to ensure residents’ medically related emotional and social needs are identified, addressed, and maintained on an individualized basis.
Key Responsibilities
Plan, organize, implement, evaluate, and direct all social services programs and activities
Ensure compliance with federal, state, and local regulations governing social services in long-term care
Develop and implement quality assurance programs and corrective action plans as needed
Coordinate social services with administration, nursing, medical staff, consultants, and other departments
Ensure accurate completion of documentation, reports, progress notes, and assessments
Participate in resident assessments, care planning, discharge planning, and community coordination
Act as liaison between residents, families, staff, and community agencies
Provide residents and families with information on community resources, support agencies, and financial assistance
Assist residents in the absence of family or responsible parties (e.g., burial arrangements)
Maintain incident and accident reports related to behavior management and elopement
Foster open communication and strong relationships across all disciplines
Quality Assurance & Resident Advocacy
Assist in developing and maintaining preliminary and comprehensive psychosocial assessments and care plans
Arrange and participate in care plan meetings with residents and families
Encourage resident participation in care planning and decision-making
Assess resident behavior and psychosocial needs; identify risk factors and monitor response to interventions
Implement and support behavior management strategies while promoting resident dignity and rights
Ensure social services staff are aware of and following care plans
Review and revise care plans at least quarterly or as required
Meet regularly with social services staff and collaborate with department leaders
Maintain ongoing quality control and continuous improvement efforts
Qualifications
Bachelor’s Degree in Social Work or Human Services field required
(e.g., Social Work, Sociology, Psychology, Rehabilitation Counseling)
Must meet federal and state certification/licensure requirements for the facility
Previous experience in a healthcare or long-term care setting preferred
Strong interpersonal, communication, and advocacy skills
Ability to interact tactfully and positively with residents, families, staff, visitors, and regulatory agencies
Patience, empathy, professionalism, and a positive attitude toward the elderly and disabled
Must be at least 18 years of age
Ability to speak, read, write, and understand English
Ability to perform essential job functions and meet physical and sensory requirements
