Overview Miami Jewish Health is one of the largest providers of healthcare and living options for aging adults in the Southeast. We operate Florida PACE (Program of All-Inclusive Care for the Elderly) Centers, the largest PACE program in Florida, which serves as health plan, healthcare provider, and social center designed for older adults with complex medical needs. We have four adult day health centers that serve participants in Miami-Dade and Broward counties. The following Job Description will also apply at Broward Pace Program sites.

Job Summary The Community Outreach Specialist in Training supports outreach and enrollment growth by assisting in the execution of outreach strategies, referral development, lead follow-up, and community engagement activities. The role carries defined performance expectations while participating in a structured training plan focused on outreach strategy, relationship management, CRM utilization, and PACE program knowledge.

Essential Job Functions

Supports the execution of the organization’s outreach strategy to drive referrals and enrollment growth

Attends networking meetings, community events, and referral source visits as assigned

Assists with building and nurturing relationships with ALFs, community organizations, and professional referral sources

Conducts planned and drop-in visits within assigned service areas

Supports presentations, exhibits, and community education initiatives

Tracks outreach activity, referrals, and relationship development in the CRM

Assists in evaluating outreach effectiveness and ROI of time spent versus referrals gained

Spends a significant portion of time in the field, with remaining time dedicated to follow-up, planning, and coordination

Supports follow-up on referrals generated through outreach and marketing efforts

Assists with nurturing prospective participants and families through education, tours, and consistent communication

Supports the sales process from initial inquiry through application readiness

Coordinates closely with Enrollment Specialist to ensure proper documentation and smooth handoffs

Conducts follow-up calls, schedules tours, and assists with community or home visits as appropriate

Accurately documents all interactions, activities, and outcomes in the CRM

Meets defined daily, weekly, and monthly activity expectations related to calls, follow-ups, and outreach support

Supports the Business Development Director in executing outreach and growth strategies across assigned markets

Provides field and administrative support for community events, presentations, and referral initiatives

Serves as a support resource to the Outreach and Enrollment teams by reinforcing follow-up workflows, communication, and documentation

Supports special outreach, growth, or market-specific projects as assigned by leadership

Job Requirements Education Bachelor’s degree required (Healthcare, Communications, Marketing, or related field preferred)

Experience 3 to 5 years of experience required; Healthcare, Admissions, Marketing, Sales, Customer Service, or related field preferred. Knowledge of the Florida Medicaid system is a plus.

Licenses/Certification Valid Florida driver’s license and auto insurance required. Medical Terminology Certification preferred.

Abilities Required

Must possess critical thinking and organizational skills

Must have strong communication and interpersonal skills

Must be able to work effectively in a team environment

Must be able to work autonomously and reach goals based on budgets

Must have computer skills, Microsoft Word experience required. Experience using a CRM preferred

Multilingual in English, Spanish and/or Creole preferred

Normal working condition with adequate lighting and ventilation
