Key/Essential Functions & Responsibilities
Receives and reviews referral packets for persons with I/DD to various YAI services including residential, day habilitation, community habilitation, respite and other programs based on regional needs to determine vacancies that may be appropriate.
Conducts pre-screenings of persons with I/DD virtually or in-person across assigned region(s) including in hospitals, nursing homes, schools and other non-YAI settings and uses understanding of I/DD, eligibility for services and person-specific needs to provide effective screening, referring people to YAI services for which they may be qualified and that meet their needs in accordance with all applicable protocols and policies.
Coordinates and participates in tours and in-person screenings for people seeking services with their circles of support and appropriate regional program staff.
Collaborates with regional program management throughout the screening process to track progress, facilitate contact between people seeking services and their circles of support and to ensure adherence to screening process timeframes.
Requests required documents for assessment and enrollment of selected individuals, assisting with completion or submission when needed, tracking documentation and progress and providing regular updates to all stakeholders.
Regularly collaborates with the YAI Financial Services Eligibility and Billing team to verify each applicant’s eligibility and to ensure that they have the correct Medicaid coverage and OPWDD eligibility for services received; ensures necessary enrollment codes are appropriately documented and coordinates updates to codes throughout the intake process, as applicable.
Works with leadership in assigned region to help plan for future needs based on existing opportunities and new development and to initiate discharges or internal moves of persons supported, ensuring applicable documentation is completed, submitted and vacancies are updated accordingly.
Enters, updates and maintains intake-related data and documents in electronic systems, including shared drives and databases (CHOICES, DA, etc.), in accordance with all applicable policies and procedures.
Establishes and maintains linkages with community partners (OPWDD, Care Coordination organizations, school transition coordinators, etc.) and other YAI departments (Program Services, Financial Services, Design and Resource) to facilitate timely identification of candidates and to effectively move them through the intake process.
Participates in outreach and community-based meetings to network and present on YAI service options and vacancies and to obtain referrals.
Acquires and maintains current knowledge of the vast array of services available to people with I/DD and related eligibility, benefits, entitlements, regulations and/or compliance requirements and of I/DD in general, including the OPWDD system, which may include attendance at committee meetings, trainings and/or conferences.
Participates in roster cleanup as directed by supervisor and maintains program rosters for assigned region(s) in CHOICES and DA.
Performs all other duties, as assigned.
Minimum Qualification Requirements
High School Diploma or its educational equivalent; and
At least two (2) years of experience working with the I/DD population or related experience in human or social services including assisting people in applying for services and/or exposure to benefits or entitlements; or
Satisfactory equivalent combination of education, experience and/or training.
Ability to travel throughout assigned region(s) including to non-YAI settings as required by caseload to conduct screenings, presentations or tours and to local headquarters or other regions, on occasion.
Ability to work a flexible schedule including occasional evening and/or weekend hours if required by caseload or for networking or marketing events.
Strong interpersonal and customer service skills, including the ability to accurately communicate, collect and present information, both verbally and in writing.
Ability to work autonomously and work well in groups, including building relationships and working collaboratively with people at all levels, including internal staff and circles of support.
Proficiency with computers and electronic systems, including Microsoft Office Suite and ability to quickly learn related systems including Workday, Salesforce, OPWDD CHOICES, DA and ePaces to complete essential tasks.
Highly organized, with good time management and follow-up skills and the ability to appropriately prioritize multiple tasks to meet prescribed timelines.
Preferred Qualification Requirements
Bachelor’s degree
Experience in Medicaid Service Coordination (MSC), care management and/or intake or admissions in the I/DD field
