The Director will oversee the discharge planning process, acts as an advocate to patients and families, provides ongoing training and development, ensure policies, procedures and regulatory compliance. Qualifications: * Bachelors Degree in Nursing -OR- Social Work required * Qualified candidates will have 3 or more years of Acute Care Case Management experience

* Proven leadership experience as a supervisor, manager or equivalent * Certified Case Manager (CCM) preferred* BLS Certified* Must be a Registered Nurse or Social Worker in the state of Rhode IslandInterested in hearing more?
