The State of New Hampshire, Office of the Governor, GO-NORTH Division has a full-time vacancy for Evaluation Lead.
Summary Reports to the Director of Transformation. Provides statewide leadership and coordination for performance monitoring and evaluation activities supporting the GO-NORTH Rural Health Transformation (RHT) Plan. Serves as the primary State liaison to the external academic evaluator and supports compliance with CMS evaluation and reporting requirements. Works collaboratively with internal and external partners to translate evaluation findings into actionable information to guide program improvement and executive decision-making.
Your Experience Counts Each additional year of approved formal education may be substituted for one year of required work experience and/or each additional year of approved work experience may be substituted for one year of required formal education.
Minimum Qualifications
Bachelor’s degree in public health, health policy, social work, behavioral health, public administration, or a related field required.
Master’s degree preferred.
Responsibilities
Plans, coordinates, and oversees internal performance monitoring and evaluation activities for the GO-NORTH RHT initiatives.
Serves as the primary State point of contact with the academic evaluator, ensuring alignment of logic models, measures, baselines, targets, and reporting cycles with CMS requirements.
Coordinates development, validation, tracking, and reporting of performance and outcome measures.
Analyzes and synthesizes evaluation findings and prepares reports, dashboards, and briefing materials for DHHS leadership and initiative leads.
Translates evaluation results into actionable recommendations to support continuous quality improvement and policy and program decision-making.
Collaborates with the Foundation for Healthy Communities (FHC) to support the SHA/SHIP Council in its oversight and advisory role.
Prepares council-ready materials summarizing implementation status, performance metrics, and outcomes.
Ensures SHA/SHIP Council feedback is documented and incorporated into ongoing refinement of RHT initiatives.
Facilitates cross-initiative learning and identifies trends, risks, and opportunities for improvement across the RHT portfolio.
