Community Engagement Specialist Remote: Must reside in Inyo, Mono, or Alpine Counties. Hourly pay: $27-$28.50/hr. Full‑time, entry‑level.

About Our Team Our care model meets members wherever they are—home or community. As part of the team, you will support engagement with members in a respectful, empathetic manner, addressing needs and providing feedback to improve services.

Role Description The Community Engagement Specialist links members to care and services. Key responsibilities include outreach, scheduling appointments, supporting form completion, attending community events, and maintaining partner engagement. Bilingual in English and Spanish, detail‑oriented, and passionate about community impact, you will make a tangible difference in lives.

Outreach and Engagement

Conduct in‑person outreach to members identified by health plan eligibility lists.

Complete Athena documentation for outreach attempts.

Schedule intake appointments and assist with required forms.

Attend outreach events, representing Zócalo Health professionally.

Build and maintain relationships with coalitions, CPI collaboratives, and other stakeholders.

Documentation and Coordination

Convert leads to patients using internal documentation systems.

Document all outreach attempts and outcomes in Athena and trackers.Assist with patient mailers and follow‑up calls to event leads.

Community Presence

Represent Zócalo Health at coalition meetings and collaborative events.

Ensure professional, consistent messaging and build trust with stakeholders.

Requirements for the Role Education and Experience

1–3 years of experience in healthcare, community outreach, or related field.

Experience engaging diverse communities with culturally sensitive communication.

Skills and Competencies

Fluent in English and Spanish, both verbal and written.

Strong interpersonal and communication skills; ability to build trust.

Detail‑oriented and organized.

Comfortable with technology; proficient with electronic health records (Athena preferred).

Additional Requirements

Work a flexible schedule, including evenings and weekends for events.

Reliable transportation for in‑person outreach and events.
